And would be valid for a year from date of signature. But *WOULD NOT* let me take a copy of their forms for inspection. If you feel like they are asking for something excessive or inappropriate, let them know. If they say "that's our policy" and there's no arguing with them, then you can either 1) Give them what you feel is appropriate and deal with the consequences (i.e. you probably won't get the job) or 2) Tell them what you think of them, WHY, and go elsewhere. I prefer #2 myself -- if they are seriously looking for people, they might want to get a clue as to why people are avoiding them.
