I got the C18 compiler and tried to use it with the Microchip ICD2 and
MPLAB.  When I configure the ICD2  as a programmer and debugger, I get an
error message that says I can't use ICD2 for both functions.

I know I could do this before using the CCS compiler.  I have to think that
if this was a new limitation of MPLAB 6.5 that it would really annoy a lot
of people.  Does that mean you have to buy and ICE 4000?  How do you develop
on 64 pin SMT devices?  You can't pull out the chip to program it.

I must be missing something.
