Worst case is that the component ends up vertical, connected at only one
end. Often you can see components with one end higher than the other, which
is the start of tombstoning, but both ends are soldered reliably.

Normally not a problem when using solder paste, unless as the original query
said, one pad is part of a large copper area such as a ground plane. However
I would not normally expect microstrip lines to be a problem as the copper
widths on both pads.
