I see a lot of commercial projects where extra pads are left for an ICD-2 connection. Some just leave pads, some leave a simple header, and some put the whole RJ-12 connector onto the board, especially for early debug boards. The main reason they don't all stick an RJ-12 connector there is because it takes up too much space and they don't want to burden a production design. For early prototypes, however, the ICD-2 connector is quite common.
