Fibre optic cable works well into the 100's of KV.

Yes. "O" with very long legs between primary winding and secondary.
Coupling losses are much higher, but thats the price you pay.

You also have to consider dust pathes that will develop.

Search for 'ultra high voltage' and 'artificial lightning'+ 'generator'
for ideas.
