Have a look at some data sheets for high side drivers - e.g  IR2183.
(international Rectifier)
They use a bootstrap arrangement to boost the drive voltage. Very simple -
but it  does require a diode & storage cap & reasonably frequent switching.
For "DC" operation you need a separate supply of some sort.

Does anybody have any good information about high voltage H-bridges? (about
200V @ 4A)

I was thinking about IGBT but I'm a little fuzzy about how to drive the
high
side of the bridge.
