Check out the Microchip technical brief "Gameport to USB Gamepad Translator"
(TB054, Doc# DS91054B).

I found it, and the other Microchip documents for the 16C745/65, to have all
the information needed to make a PIC based USB game controller.  TB054 also
has a good description of PC gameport hardware.
