Just one more question. MCLR is tied up to VDD only when using the
programmer or should I tied it also during normal operation?

All of the VDD's must be isolated, sorry. Otherwise the PICSTART will be
trying to pump up your onboard power supply thru the chip, and that
might damage the chip.
