When I worked for Motorola in the late 70's, they had a system using an inductive loop pickup inside the ear, with a loop worn under a jacket and mounted on top of the shoulder. The microphone was, I think, either worn on the lapel or on the sleeve. The radio was held in a "shoulder holster" type arrangement that took us quite a while to figure out how to put on! The earphone fitted well inside the ear and had a small pin sticking out of it that had to be used for retrieval.
