Well, I would think that a flesh colored "ear-bud" would be less visible than someone walking around with wires hanging out of their mouth!! :o) How about a short range RF link from the a belt-worn voice radio to a tiny RF hearing-aid-like device? Many modern hearing aids (while costing >$1000) are invisible, except upon close inspection. It might be possible to gut the amplification circuits and replace them with a single-chip RF receiver. How about a baseball cap with a speaker in it, doing the same thing as the mouth-ball idea (transferring the sound through the skull? I'm not sure that the volume required to transmit the sound through the skull wouldn't result in some noise emission as well, though. Hmmm...
