You seem to be doing everything right.  In a few cases the magic
bullet for me was a series inductor.  Just make sure that ferrite
bead offers enough attenuation at the frequency you are using.
They aren't magic, either.  Meaning if they don't work you might
consider a real inductor.

Also worth a look at the installation.  Sometimes you will spot
something boneheaded right away...
