I have mentioned before on here the definitive document from Microsoft.
Search their site for a file called fatgen102.pdf. The 102 is the revision
of the file, and so may change, but IIRC the last time I looked for it I
just searched for "fatgen".

This file is the definitive definition of the FAT file system, and contains
all sorts C code scraps on how to do the various operations, and also
details the pitfalls of some other implementations that are around, and why
they can fail.
