You might check the voltage across the LED/Load in the steady state condition to see if it is adequate to light the LED. I presume you are nowhere near the max load of 25MA but if you are, Voh is specked at 2.7V and Vol is specked at 0.8V. That would net 1.9V across the load. Also, 100K on pin 1 sounds a little (x10) high. FWIW I've always connected one side of the load to +5 as Vol is a little more load independent than Voh.
