I usually ignore updates until I see a fix for a bug that may affect me, which is pretty rare. I have been using CCS for - what, nearly three years now, I think. I have actually run into compiler bugs all of maybe twice. Last time was a bug specifically related to the 12F675 init, and the folks at CCS were kind enough to let me download the latest version with the fix even though my support had run out. I think there was one other time, but it was a long time ago and it may have been me and not the compiler. Optimzation is good, not perfect, but there's a LST file you can check for anything oddball. I have seen a lot of complaints int he past about the PIC18 version, but it seems most of that has been fixed (I have not yet used it). In general, I'd say you can expect to not see compiler bugs unless you're doing something weird. It's certainly not GNU C or anything, btu the time it's saved me FAR outweighs the time I've spent chasing the occasional compiler quirk. I have certainly not found anything better for the money. I checked out a couple of less expensive compilers and found all to have major flaws, but that was a couple of years ago and I'm sure some have improved since then. I know a couple still suck badly.
