How about the obvious, tying the LVP low with a resistor, yoy can still use it as I/O if you disable LVP in the fuses. Note that the LVP for the 18Fs is RB5, so a cable that worked with a 16F might not work with an 18F, unless both RB5 and the 16F LVP pin are connected (and the programmer pulls them low).
