Just did one myself.

Uses FDTI, 16F628, xtal, handful of passives and 1 transistor. High
voltage programs 12 18 28 and 40 pin PIC flash chips in a 40 pin ZIF.
Can also program most C series if external 13V supplied. Also has PC
control of MCLR.
