When the MOV breaks down, it carries whatever current the source will deliver. If the Joule rating of the MOV is exceeded, it will be destroyed, or seriously degraded. If not, the MOV has hopefully served its purpose, and your circuit will go on to live another day. If the tracks to the MOV blow as a result of the surge, no further protection will be afforded by the MOV. I always use the thickest possible tracks that the design will allow.
