I've always set the drill diameter and pad diameters in the package
definition.  However, I vaguely remember there is a setting you can use in
the package so that some of the particulars are overridden by the design
rules.  I remember thinking this was dangerous and always specified
everything in the package, then forgot about it.

After all, the package definition is a reflection of the mechanical
details of a part.  A specific part will have leads with specific
diameters which need to be taken into account whether the design rule
preferences like it or not.
