The amplifier circuit driving the final pass element is allowed to have some voltage gain. Think of it as a DC amplifier that may not be particularly fast but can source a hefty current (and probably not sink any at all). If it has a gain of 5, then the output can be adjusted from 0 to 25 volts with an input signal from 0 to 5 volts.
