Advanced Circuits (www.4pcb.com) still has their "$33 each" offer in effect. So your cost from them would have been $132 + UPS. I've used them numerous times and always received excellent, professional- quality boards with all the features you describe. Size can be anything up to 10" x 15", though no gold fingers or slots.
