I am not sure but I think that what happened on Apollo13 was, that an oxygen tank's internal mixer (?) malfunctioned and that something or other iced up and blew (maybe the tank proper or the plumbing). I do not think that it was the fuel cell. Afaik their problem was that part of the oxygen supply was gone so they couldn't run the fuel cells (did they have more than one ?) and had to shutdown everything for as long as possible. I don't really buy the story about things being dropped and malfunctioning when the items in cause are built to withstand 30+g's for 15 minutes or so.
