All in series? I set mine up as follows, with ICSP working (so far) MCLR ties directly to ICSP connector MCLR to Cathode of Diode Pullup resistor from Anode of diode to +5V Small cap from Anode to GND. This was on a 16F877, and ICSP has been working great so far. The small cap may be optional, I had the space so I included it.
