If your supply line inverts polarity on power up or any of your pins see negative voltages very weird things can happen. This might happen if you power on cycle is so bad as you describe. One option to test if this is the case is to make a hardware power up delay of the pic's supply. Easily done with 1 or 2 small transistors and passives.
