All you need to do is make sure the "Enable Debug Mode" checkbox is cleared in the ICD options dialog. Then after programming you should be able to disconnect the ICD from the target and go. One thing to watch out for: when using the ICD it will drive the MCLR line appropriately. You should make sure that MCLR will be high on the target with the ICD disconnected.
