Ken, In the PCB editing panel, go Place > Pad, then hit the TAB key, which will bring up the Pad Properties edit box. In here, edit the pad properties to your liking, selecting Shape as Rectangle, and Layer as MultiLayer, and input suitable thru-hole size. File > CAM Manager brings up a CAM wizard (if not previously set up) where you define your CAM requirements, alternatively right-clicking once the CAM panel has been opened allows you access to CAM options. IIRC, there is a tutorial provided with the package, or on the web site www.protel.com, which will help you familiarize yourself with the basics. Good luck.
