Well I think the biggest gotcha you have is the MPLAB 5.50, buginfested
piece of ***** !!
I've had serious issues with the programming through 5.50. Specifically
the fuses were incorrect, bascilly all of them were random, made programmed
part totally unusable, however if the program were read back (write protect
off) and
the fuses were 'corrected' ( in 5.2 version ) re-programed another part
then it works, so it seems it's only the fuses that gets mangled.
Read back the fuses of your programmed part in another version
of mplab ( pre 5.50 ! ) and verify if this is your case.


Apart from that, there are a few things that could be noteworthy:

-Reset line
-Powersupply ( ICE can be setup to use 'internal' power )
-Clock ( ICE can use own crystal )
