I found one at DigiKey on my first search. I didn't check what the form factor is, but it is only $US 0.91 in single quantity. It is item number 300-5020-ND at http://www.digikey.com . It might be too much trouble to order from the US, but you can at least get the manufacturer and specs from that and try to buy it locally.
