As I recall, a member of the Forth Interest Group (David Jaffe, I believe) did a lot of work on this with Stanford Hospital or perhaps with one of its clinics. He used Polaroid ultrasonic ranging units to sense head movements which then controlled an electric wheelchair's speed and direction.
