I've always unplugged my ICD for free running. But I noticed today that I can run the PIC without the debug code and the ICD still plugged in. After you program the PIC, reset it from MPLAB (it takes longer to reset than with debug code) and the PIC starts running normally. I guess the reset releases the MCLR.
