You can almost do it in MPLAB. All you need to do is Load the HEX file using file->Import->Import_to_Memory menu item. And then, you save the disasm file by using "file->Export->Export_memory menu item. This will bring up a save dialog box. In the list of file types combo box, select Disasm files. And save it.
