I have no problem with clamping the body, in fact with the poor design of the TO220, it's probably the very best thing you can do. In a production situation you might be able to get away with the system described with control on the torque (maybe by making the bar flexible, and/or by using auto torque screwdrivers) or by using a 'V' rather than a dimple to spread the high concentration of force on a brittle material.
