What you raised are all good questions, mostly for which there are currently no good answers. Science works incrementally, a little step at a time. Theories change daily. Egos get in the way. That's human nature. It takes a long time and a lot of work to tackle these issues. It is only since the Hubble Telescope - just a decade now - that we are really beginning to see the "big" picture concerning the universe. This one instrument alone has widened the possibilities immensely. Maybe all of these open questions are better viewed as cause for further conjecture, rather than cause for asking why all of the answers aren't already known.
