As part of our ISO9000 procedures, we DO have a rather complex naming scheme and storage hierarchy to all our documents. However, this is all hidden behind a database-like front end that is accessed via web pages. A relatively simple database can be handy for this sort of thing (and I suspect that documents are most widely access via ... search engine :-) As semi-designated curmudgeon, my own documents tend to be plain ascii files with names like "udptn-fs.txt" and "udptn-ds.txt" for functional and design specs, respectively.
