From Digi-Key, they list an 8-pin 12C508A OTP for $1.58.  This is the
cheapest I've seen in single-piece quantities.

This only has 512K of EPROM and 25 RAM bytes with 5 I/O lines.

They also sell a 12C509A OTP for $1.63 which has 1K of program memory with
41 bytes of RAM, but still only 5 I/O lines.
