Proper bypassing is critical.
Also, have you checked the voltage? I wouldn't trust a wall-wart unless I
had metered it.

In order for any micro to run right, you have to have power, inside the
voltage spec range at all times, and enough current to meet peak demands.

You have to have supply bypassing at the micro.

You have to provide proper reset input (mclr on pics)

You have to provide proper external crystal and caps (unless using internal
or external R/C oscillator)
