Well there's two possibilities: It was converted from RTF. Last I knew, RTF doesn't have the ability to include images, or perhaps the conversion program didn't take these extensions into account. Secondly, many books have a second or third party create/touchup/modify/digitize figures, pictures, and other image material. It may be that the author did not ever hold copyright to the majority or all of the images, and therefore cannot release them.
