My brains and Word (any other set of text/graphics/programming would be fine too, for instance vi + pen/paper + C). My current company likes Doors (a requirements managing tool) but I perefer Word (with a some ad-hoc of word basic code) and Access. But I must say that I probably never did a huge software project (with let's say 100+ developers). For single-person development (for me up to 10 - 30k lines) any technique (or no technique at all) will work. For larger projects (more people) the company will often have a preferred (or mandatory) technique, so it is best to use that (save your energy for making small adjustments, not to fight the system).
