I have a PIC-related (yes, several in the system) which I need to change from a
radio-button or pulldown list to a checkbox.  The program is changing from a
single value to bitmap input.

The basic form exists, but does only one entry at a time.  What I need to do is
learn how to give an HTML page which allows the user to select multiple entries
as checkboxes and send that to me when they hit submit.

ie:

REPORT THESE ERROR CONDITIONS:

O  System reset
O  Nuclear blast in close vicinity
O  Al Gore learns to dance
...
O  Rush says "No" to another helping

This is a 32-bit field I'm going to be populating on the PC.  I just need to see
how to get the HTML form code to send the right thing to my CGI app.   And by
"right" I only mean "parse-able" and "not way too tough."

Are you confused now too?  Good, then you can explain it to me!
