I have had to do this before and I think it cost around $50 at the time
to create a one off from a PCB manufacturer. (about 6 inch square which
gave me 36 small PCBs) Pick a small company and offer them cash in hand.
As a bonus they usually make more than one to be on the safe side and
you could end up with these for free.
