  Misunderstanding of "Safe Mode".  Very few things ever work in "Safe
Mode".  It has nothing whatever to do with testing applications, rather
cobbling unstable Windoze configurrations (and then, with considerable
luck).

  Try de-installing and reinstalling whatever contained that DLL.
