The only match I could find is from AKM Semiconductor
( http://www.akm.com ).  It's a 4096 Bit Serial CMOS EEPROM.

BTW, an invaluable resource for finding stuff like this (part number with no
manufacturer info) is at the EE Design Center: http://www.questlink.com .
They even have data sheets for most things.
