Contact the U.S. Department of Commerce, and ask them for some "Shippers
Export Declaration" forms, and the booklet "Correct Way to Fill Out the
Shipper's Declaration".

You may also (depending on the item being shipped) need the purchaser to
fill in an end-user certification form.
