The firmware resides in the 17C44 inside the PICSTART Plus. You need to re-program that device (if it is windowed), or replace it with a 17C44 programmed with the firmware .HEX file that's included with MPLAB v3.99.21. You can get an upgrade kit from your distributor or do it yourself.
