Power Convertibles (A Burr Brown Company) makes a series of tiny,
economical dc-dc converters.

Model HPR111 converts 12V to +/-15V at output max of +/-25mA, 80%
efficiency.  The size measures 20x10x6mm (LxHxW).   I bought them at
around C$14 each.  They can be reached at 1-800-548-6132.  I think they
are one of the smallest available and at an affortable price.
