A few years ago I bought a horribly expensive module from
Pico Transformers to do just exactly this.  It did work.
You may want to look at Maxim et. al. for better controller
chips nowadays, but the down side to all those is that you
have to wind your own transformer.  If anyone knows of
an off-the-shelf transformer that mates well with one of these
chips, I'd certainly like to hear about it.
