I have found error in the flow chart (figure 2 -Initialization of LCD module
at page 3 of the AN) in the display on command. This command should be:

 RS  R/W DB7 DB6 DB5 DB4 DB3 DB2 DB1 DB0
  0    0   0   0   0   0   1   1   0   0

The AN states wrong numbers for both 8bit and 4bit modes. If you run the
initialization sequence, you will never have the display back on. The code
in the appendices is correct, however.
