John: There are LOTS of PCB designers in the world... If you've already chosen a board house to produce your boards, ask them if they have a PCB designer on staff or if they can recommend one. You might also find someone here on the list who'll be willing to do your board. Prices are usually quoted "per hole" (or "per pad" for surface-mount boards)...
