MPLAB (current version 3.31) includes a disassembler.
... AND - It's free!

Method:
In MPLAB read the chip or load the HEX code  >File>Import>Download to Memory.
Open the Program Memory Window >Window>Program Memory
Click on the [-] in the left corner of this window - it gives you the option of
"Hex Code Display", "Machine Code Display", "Disassembly Display".
