Has anyone had problems with the IBF, OBF and/or IBOV flags getting set
erroneously when operating the parallel port in polled mode (not using
the PSPIF bit/interrupt) in the 16C64?

I see the IBOV flag getting set when the host is reading the PIC's
output register (in a polling loop), than when the host writes to the
PIC, the new data is not transferred to the PIC's input register and the
PIC ends up with the same data from the last legitimate write.
